At Rise Up Care and Support, we know how important it is to feel socially connected. Our Social Support services are designed to reduce isolation and help participants build lasting relationships. Whether it’s joining community groups, attending events, or simply spending time with a companion, our team provides safe, respectful, and encouraging support. By creating opportunities for meaningful interaction, we help participants improve confidence, social skills, and overall wellbeing.

You may be eligible if:

  • Your NDIS plan includes Core Supports or Capacity Building funding
  • Your NDIS plan includes Core Supports or Capacity Building funding
  • You require assistance with daily tasks, activities, or community participation
  • You want to strengthen independence, confidence, and wellbeing

Age requirements:

Our services are generally available for participants aged 7 to 65 under NDIS funding. Children under 7 can access support through early childhood partners. Participants over 65 may be eligible for aged care services or alternative funding programs. If you are unsure, our team will guide you through the right pathway.
